7/21/2023 EnchantaVaughn Approved Upon arrival, client was observed coming into the office setting in a positive and respectful manner. Client hugged QMHP before going into his age group. Client debriefed about yesterdayâs events and which event he liked most and couldnât wait to do again today. Client utilized his break to practice relaxation and breathing techniques. Client was supported with resting and preparing for the next activity. Client realized that the next activity was a karate lesson and he did not want to participate. Client expressed negative comments about the instructors and lesson making it hard for him to concentrate and focus. Client sat out of a few activities today (e.g. musical chairs, art, kickball, and tag). Client was only interested in other team building games, such as hide and go seek and rock paper scissor. (EOD) Client stayed with QMHP for most of the day. Client had a difficult time following rules and listening to authority figures. Client was off task all day and ignoring instructions. Client was seen walking away from staff and disrupting others who are trying to learn. Client was encouraged to leave people alone and give them their personal space. Client displayed frustration and began acting out and displaying inappropriate behaviors. Client needs improvement with controlling his anger and handling his emotions. QMHP observed client while in the classroom in efforts to determine the need for intervention. QMHP provided a positive hand gesture such as a thumbs up in efforts to show approval of clients current actions. QMHP did not have to redirect client. QMHP did not have to intervene with clients behaviors as he demonstrated adherance to classroom rules and expectations. QMHP provided a simple hand gesture to encourage client to continue with the positve classroom behaviors. QMHP provided strategies such as positive reframing. QMHP assisted client with practicing reframing negative thoughts with more positive thoughts in efforts to reduce impulsive reactions throughout the day. QMHP was informed of the clients behaviors. QMHP reviewed the reported behaviors with client in efforts to allow him to express his point of view. QMHP encouraged client to list alternative behaviors he could have participated in. QMHP will explore positive self talk, use instruction modeling, and increase social communication skills with positive reinforcement role play activities. QMHP will educate client on behavior modification to help client identify what behavior he need to decrease and avoid daily. QMHP will utilize behavior modification system to aid the client with following directions given by authority figures. QMHP will meet with client daily to discuss progress and expectations that are working and not working in camp. QMHP processed appropriate ways to approach situations that he felt were unfair towards him in efforts to reduce his defiance and increase coping skills. QMHP reinerated clients ISP objectives in efforts to process the necessity of positive behaviors in the classroom setting. QMHP encouraged client to prepare for the next activity by cleaning up and waiting for permission from staff before leaving his assigned area. (EOD) Client was able to reduce negative behaviors and focus on the camp activity. QMHP will continue to assist client with managing anger and frustration by reviewing and modeling effective coping skills. QMHP encouraged the client to respect his instructor. QMHP praised client for his cooperation inspite of not wanting to do the activity. QMHP praised client, gesturing for him to remain on task. QMHP cheered on client as he decided to challenge himself with new ideas. QMHP supported client participation and recognizing his own behaviors and correcting himself without guidance. Client expressed positive self talk, modeled positive behavior, and good communication skills during role play activity. Client learned to avoid negative and defiant behavior and comply to rules. Client was seen following directions. Client joined session to utilize and learn new interventions and strategies. Client agreed to stay focus and have a good day. Client displayed a lot of low self esteem with interacting with other peers. Client demonstrated how to verbalize his feelings without yelling and being aggressive. Client was able to speak calmly and demonstrate his improved verbalization skills while conversing with other mental health professional. Client did not display any outburtsts during the role play exercise. Client appeared to be learning from his mistakes and correcting them throughtout the day. Client responded well to praise and encouragement. (EOD) Client saw that he was the only one out of line and went back to his spot. Client worked well in groups and was seen sharing his supplies with his peers during a coloring activity. Client was happy to be on track and displayed cheerful emotions. Client said he was aware of his expectations and camp rules. Client expressed a good temper when given prompts to follow. Client responded well to praise and support. Client presents appropriate interaction in camp while participating in all activities. Client was proud of himself, accepting QMHP compliment by saying thank you.
